Your First Appointment: What you Need to Bring

We look forward to meeting you at your first appointment. In order to make the most of your time with the counselor there are a few things you should bring with you.

Print and Sign the Intake Forms

Please find your counselor’s name below and download their Intake Forms. Print the forms, read them, sign them, and bring them with you. Your counselor will review the forms with you and answer any questions at that time.

Bring a Credit Card

You can pay for services with cash, credit card, or check. However, a credit card must be kept on file for fees related to missed appointments and late cancellations. We accept American Express, Visa, MasterCard, Discover, and JCB.

For counseling children, bring a Divorce Decree if…

If you are divorced and want your child to attend counseling, you are REQUIRED to bring a copy of your divorce decree showing you have the right to place your child in counseling. The counselor will retain the copy of the divorce decree for our files.

Your counselor will answer any questions from you at the beginning of your first session. We will see you soon!
